

Office Hours & Appointments:
To make an appointment please call (828) 837-7513 or Toll Free (888) 837-7513.
Our normal office hours are 8 am - 5 pm, Monday through Friday. We are closed
between noon and 1 pm for lunch. If you are visiting us for the first time, please arrive 15
minutes before your scheduled appointment to allow time for registration.
Please make appointments so my staff can schedule the time needed for your concerns.
If canceling an appointment, please allow 24 hours notice. Canceling without
appropriate notice would cause other patients to be denied an appointment when they
might otherwise be seen. Your cooperation and consideration in these matters would
be appreciated.
We make every effort to schedule your appointment within 10 days. If you have an
emergency, we will try to see you that day, if you call in the morning.
There are occasions when emergencies or other unforeseen circumstances will interfere
with our office appointment schedule. Our staff will make every effort to notify you , if any
significant delays are anticipated.
Please bring the following to your appointment:
· Medical
insurance card(s)
· All of the prescriptions and/or over-the-counter medications you are currently taking in
their original containers
· Your
completed registration forms.
· Relevant
medical records
· Any
films or lab results pertinent to your condition
Telephone Calls:
Our office telephones are very busy and you will occasionally be asked to hold for a brief
period. Please be patient with us. We receive many calls during the day and it is unfair
to the patients who have scheduled appointments to continually interrupt me for
telephone calls. Therefore, the receptionist will usually take a message and your call will
be returned at the end of the day. Please inform the receptionist if your problem is
urgent and she will let me know.

severity of the condition
· How
long you have had the symptoms
· The
course the condition seems to be taking
Emergencies:
If you have an emergency situation during office hours, please call first, so you can
receive as prompt treatment.
Please do not leave a message on our voice mail in an emergency situation.
After regular office hours, for an emergency situation, please go to the Emergency Room
at either Murphy Medical Center or Union General Hospital and provide them my name to
ask for advice and any needed assistance.
Prescriptions:
All routine prescriptions and authorizations for renewal should be requested during your
office visit. If you run out of a medication, please call the pharmacy and the pharmacist
will call our office for an approval on the refill. We normally will only refill a weeks
medication to allow you to schedule your office appointment. You cannot have
medication refills for a condition not currently being treated. Normally, antibiotics are not
a refillable prescription .
Due to the significant number of insurance companies requiring pre-authorization for
prescriptions, we cannot make telephone calls, accomplish insurance forms, or
otherwise assist with your prescription. Please inform us of the type of insurance plan
you have.
Financial Policies and Procedures:
We participate and will file most insurances for billing purposes. We ask that you have
complete insurance information available for each visit. Your insurance company may
also require you to pay a co-payment at time of service. We request payment at time of
services for any charges that are not covered by your insurance plan.
All new patients are required to read and sign our Financial and Payment Policy that
explains are billing practices. Patients without insurance are required to pay their
charges at the time of services except for major procedures. If a payment plan is
needed, a financial hardship form will be accepted to determine the payment
schedule.
